Skilled Nursing

For patients who need intensive rehabilitation after surgery or hospitalization, ACMC offers inpatient skilled nursing services.

ACMC's Skilled Nursing Unit can ease the transition to independent living by returning residents to their highest level of function, while preparing them to return to their previous living arrangements.

Residents of the ACMC Skilled Nursing Unit can be closely followed by their acute hospital physicians who are familiar with the circumstances of their hospitalization. Residents and physicians also have immediate access to state-of-the-art diagnostic services. This means changes in a resident's condition can be rapidly assessed, and rehabilitation care will meet the resident's individual needs.

Eligibility for Admission

The Skilled Nursing Unit accepts Medicare, as well as many private insurances. A Medicare patient must meet the established Medicare rule of three midnights in a hospital setting before admission to the Skilled Nursing Unit. The admissions department works with your insurance company to determine eligibility and benefits prior to admission. Patients referred to the Skilled Nursing Unit must also meet the guidelines for skilled care. Some examples of skilled care include:

  • Physical therapy.
  • IV antibiotics.
  • Occupational therapy.
  • Wound care.
  • Speech therapy.

Referral Process

Prior to, or during hospitalization, a care manager will be notified of a patient's post-discharge needs. The care manager will then offer the patient a referral to his or her choice of skilled nursing facilities, including the ACMC Skilled Nursing Unit. When the patient chooses the ACMC Skilled Nursing Unit, the care manager will refer that patient to the Skilled Nursing Unit's admissions coordinator. The admissions coordinator evaluates patient information and facilitates the admission process.
