Conneaut Family Health Center’s family medicine providers offer comprehensive, personal health care – from routine checkups and preventive health, to diagnosing and treating common illnesses and injuries. ACMC Express Care

If you have a minor illness or injury, save time and money with ACMC Express Care. Visit Express Care for:

  • Cold and flu symptoms.
  • Conjunctivitis.
  • Minor back pain (without history of trauma).
  • Minor bumps and cuts.
  • Seasonal allergies.
  • Skin rashes.
  • Simple sprains and strains.
  • Infections of the sinus, upper respiratory or urinary tract.
  • Ear, nose and throat.
  • Shoulder pain (without history of trauma).
  • Insect bites.
  • Cellulitis.
  • Influenza.
  • Styes.
  • Ringworm, shingles and impetigo.
  • Cold sores.

A visit to Express Care costs less than a visit to an urgent care facility or the Emergency Department. Co-pays will be similar to a physician office visit. Express Care is a walk-in service for patients age two and older. You do not have to be a patient of ACMC or Ashtabula Family Health Center to use Express Care. Hours at the Conneaut location are Monday through Friday, 7:30 a.m. to 7 p.m., and Saturday 9 a.m. to 2:30 p.m.

Lab Services

The ACMC Laboratory provides accurate, timely, convenient, and affordable testing and blood collection services to physician offices and patients. Stop in Monday through Friday, 7 a.m. to 3:30 p.m. and Saturday 8 a.m. to noon

Digital X-Ray

ACMC offers digital X-ray - a non-invasive procedure which creates a digital image of the inside of the body - Monday, Tuesday, Wednesday, and Friday from 7:30 a.m. to 5 p.m.; Thursday 7:30 a.m. to 6 p.m.; and Saturday, 8 a.m. to noon. To schedule an appointment, call 440.997.6686.

2-D/3-D Digital Mammography

Mammography is the gold standard test for diagnosing breast cancer. It uses digital 2D or 3D technology to image the breast. 2D mammography uses low-dose x-ray imaging along with computer-aided detection (CAD) to create an image on a computer screen. 3D mammography creates a series of three dimensional images of the breast. Patients are asked to check with their insurance provider to determine if 2D or 3D mammograms are covered or recommended. Physical Therapy

Physical therapists at ACMC team with licensed physical therapy assistants to deliver exercise-based treatments. Other modalities may be employed to maximize patient outcomes, including ultrasound, electrical stimulation, cold or heat therapy, manual therapy, joint mobilization, aquatics, resistance training, and education. Patients receive therapy as outpatients, hospital inpatients or skilled-nursing residents. The goals of physical therapy usually include restoring a patient's prior level of function and alleviating pain.

Physical Therapy is available at Conneaut Family Health Center Monday and Wednesday, 7:30 a.m. to 5 p.m.; Tuesday, 7:30 to 11 a.m.; and Thursday, 7:30 a.m. to 2 p.m.
